Number of only unduplicated Santa Ana residents to be served by the funded program, named above, <br />during the 12-month contract period. <br />lQPERSONS <br /> <br />Program and Funding Description <br /> <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month <br /> <br />contract period. <br />Mercy House operates two transitional programs for the homeless. Joseph House is for homeless single men, including veterans and <br />emancipated youth, Regina House, our transitional program for single women with children, house seven mothers and their kids. <br />The transitional shelters offer supportive services, including individualized case management. The goal for the Mercy House clients- <br />is to end the homelessness of all who enter into our system of care. In order to achieve this goal the programs provide supportive <br />services through three programs. They are: <br /> <br />1. WeIlness Program - where the objective is to improve the client's mental and physical health conditions. <br /> <br />2. Planning for Success Program - where the objective is to improve the material and financial self-sufficiency of the clients. <br /> <br />3, Learning Matters Program - where the objective is to improve the client's quality of life because of increased knowledge and <br />modified behavior. <br /> <br />IV. Activities funded by this grant - How will CDBG funds be used for the funded program? Narratively <br />describe the budget for the funded program illustrated in Exhibit B and B-1. <br />CDBG funds will be used for rent, communications, utilities, insurance, and program expenses. <br /> <br />Exhibit A <br />Page 1 of2 <br />
