Description of Work - In the space below, describe the program to be funded during the 12-month <br />contract period. <br />The donation based Home Delivered Meals Program assists frail, at-risk Santa Ana older adults to reamin in their home following the <br />on-set of a chronic or acute physical and/or mentally debilitating illness. Three meals are prepared and delivered Monday through <br />Friday. Under the supervision and guidance of our Registered Dietician, all nutritional needs are addressed. Goal- Feedback <br />Foundation projects to serve 420 unduplicated homebound older adults with meals and other suppport servies in 2006/2006. <br />Feedback Foundation anticipates serving 205,000 meals to Santa Ana elders during 2006/07. <br />IV. Activities funded by this grant -How will CDBG funds be used for the funded program? Narratively <br />describe the budget for the funded program illustrated in Exhibit B and B-1. <br />The primary source of funding for the Home Delivered Meals Program is the Older Americans Act. Over the course of the last ten <br />years, funding for the Older Americans Act has increased by 16%. Howevere, those needing meals has increased by 44% indicating <br />an actual decrease in funding. We are seeking fmancial support from the City of Santa Ana to assist us in closing the gap between <br />fuding and costs. Because this program targets low income and minority residents, donations received from the participants do not <br />begin to cover the costs. The average donation for the Home Delivered Meas Program in Santa Ana is $0.38. All CDBG funds used <br />for this program will go strictly to the costs of raw food. <br />Exhibit A <br />Page 1 of 2 <br />
