In addition, self- <br />assessmentevaluations and qualitative interviews by staff determine progress. Annually an assessment of the <br />program's effectiveness indicated by strength of community support, quality of collaborative partnerships, and <br />confidence of community and government social services to offer referrals and form collaborations is also conducted. In <br />addition, ongoing systematic evaluation is conducted by the leadership, staff and the volunteer Boardof Directors to <br />identify corrective actions needed to improve the program. <br />Exhibit A-1 <br />Page 3 of 3 <br />
