A .Organization and Credentials <br />Provide a synopsis of the firm's qualifications and past experience. <br />B. Monument Preservation <br />Provide a detailed description of your procedure for recovery and <br />perpetuation of survey monuments endangered by the proposed <br />construction, including preparation and submittal of the necessary records. <br />C. Construction Staking Approach <br />Provide a description of each task in your construction staking approach <br />from the receipt of the survey request to the submission of the staking <br />records, including the personnel categories assigned to each of your <br />specified tasks. <br />D. Staffing <br />The consultant shall identify their Project Manager as well as other key <br />personnel to be assigned to the project, their qualifications, education, <br />representative experience, and their capability to explore and resolve <br />problems. <br />19 ] ADDITIONAL INFORMATION AND COMMENTS <br />The contents under this heading are left to the discretion of the consultant. <br />Material shall be pertinent to the proposal, but not otherwise requested in the <br />RFP. <br />20 ] CRITERIA FOR SELECTION <br />The response to this Request for Proposal should contain documentation of <br />consultant's credentials and expertise in this field. Proposals will be evaluated on <br />the basis of the qualifications, abilities, and responsiveness of the proposing <br />firms. <br />All proposals received will be fully reviewed and rated by an Evaluation <br />Committee. Following this, the Cost and Price for the best proposals will be <br />opened. The ultimate consultant selection will be based upon both technical <br />merit and cost competitiveness. <br />21 ] THE CITY'S AFFIRMATIVE ACTION PROGRAM <br />The attached "Certification of Non-Discrimination by Contractors" shall be <br />completed by each submitting firm and included in the proposal (Attachment "B"). <br />22 ] INSURANCE REQUIREMENTS <br />A sample certificate of insurance is enclosed for the consultant's reference <br />(Attachment "C").
