<br />S. Collect. analyze and report to Youth Council the results ofthe Customer <br />Satisfaction Survevs <br />T. Create and implement a continuous improvement project based on the results of <br />the Customer Satisfaction Survey <br />U. Be available and prepared for monitoring by Federal, State and SA WIB staff <br /> <br />4. FOLLOW UP SERVICES: Follow-up services v."ill be a coordinated effort <br />between the W/O/RlK Center Service Navigator and the youth program operators. <br />The purpose of follow-up is to determine a participant's employment and educational <br />status after exiting the WIA program and to provide additional assistance if needed. <br /> <br />5. BUDGET: The W/O/R/K Center acting as the "Provider", shall provide all of <br />the services discussed herein as well as those set forth in Exhibit A. for the monetary <br />amount set forth in its budget. Said Budget may annually be updated as approved by <br />the WIB. <br /> <br />6. DURA TION: This MOD shall remain in effect until a written cancellation IS <br />submitted by either party giving a 30-day advance notice. <br /> <br />7. AMENDMENTS: Either party may propose amendments to this MOD at any <br />time by providing written notice to the other. Amendments to this MOD shall require <br />the approval of the authorized signatory for the WIB and the Deputy City Manager <br />for Development Services. on behalf of the W/O/R/K Center. <br /> <br />8. DISPl:TES: The parties shall first attempt to resolve all disputes informally. <br />Any party may call a meeting of all parties to discuss and resolve disputes. Should <br />informal resolution efforts fail, the dispute shall be referred to the Deputy City <br />Manager for Development Services to act as mediator, to attempt to resolve the <br />dispute by holding an informal hearing with presentations by both parties. If the <br />Executive Director's resolution efforts fail, any party may file a grievance with the <br />Deputy City Manager for Development Services for review and hearing. The parties <br />agree to be bound by the final determination resulting from that procedure. All costs <br />associated with the grievance procedure shall be borne by the losing party. <br /> <br />9. SEVE~i\BILITY: If any part of this MOD is found to be null and void. or is <br />otherwise stricken, the rest of this MOD shall remain in force. <br /> <br />10. HOLD HARMLESS: Each party to this MOD agrees to indemnify and hold <br />harmless the other parties, their officers, agents, employees, and volunteers from and <br />against any and all loss or damage, and from any and all suits, actions and claims <br />filed or brought by any person or persons arising out of acts or omissions of the party <br />or its officers, agents, employees or volunteers in the performance of this MOLT. <br /> <br />11.
