Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br /> <br />CMS staff will conduct an intial in-depth intake that will encompass a thorough needs assessment to determine if <br />the individuals or families are in need of any other services available through our programs or through a referral <br />source. Our staff will attempt to accommodate any of those needs for which we have resources and will make a <br />referral to any other agency or organization for needs that are not readily accessible through our site. <br /> <br />Thereafter, within three to six months after service delivery, we will conduct follow-up contacts, either by <br />telephone, written communication or personal interview, to ascertain the clients' current status. In many <br />instances, because many of the housing clients also participate in our emergency food bag and clothing <br />programs, staff will conduct follow-up case management services on subsequent visits to our site. The <br />subsequent visits will also allow staff to assess whether the clients have had any crises situations since they <br />received assistance or are currently in a crisis situation that we can remedy. <br /> <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12-month <br />contract period per quarter.
