FY 2025-26 CDBG Program Funding Plan <br />Program Summaries <br />Nonprofit Public Service (subject to 15% Public Service cap) <br />Project Title Organization Funding Description <br />AIDS Services <br />Foundation OC (dba <br />Radiant Health <br />Centers) <br />HIV Care Services $34,860 <br />AIDS Services Foundation OC (dba Radiant Health Centers) will <br />provide low-income residents living with HIV in the City of Santa <br />Ana with wraparound HIV care services. Participants will receive <br />the support and resources needed to remain in medical care, <br />maintain their health, and achieve viral suppression. <br />America On Track <br />Brighter Futures <br />for Children of <br />Prisoners <br />$34,860 <br />Children of prisoners deserve the chance to discover a brighter <br />future, which is why America On Track provides them with trained, <br />caring mentors, STEM workshops, a “College is For Me Too!” <br />camp, free books, and special outings, as well as nutrition seminars <br />and wraparound services for the families. <br />Community Health <br />Initiative Orange <br />County <br />Community Health <br />Access Program $34,860 <br />CHIOC helps City of Santa Ana residents to access health and <br />social service programs by providing outreach, education, <br />enrollment, and case management services to vulnerable <br />populations. We strive to ensure that clients acquire, retain, and <br />utilize community resources effectively. <br />Community Legal <br />Aid SoCal <br />Santa Ana <br />Domestic <br />Violence <br />Prevention Project <br />$34,860 <br />Community Legal Aid SoCal (previously known as The Legal Aid <br />Society of Orange County) will provide free, holistic legal <br />assistance to Santa Ana survivors of domestic violence to help <br />them escape abuse and break the cycle of violence. Priority is <br />given to service in the areas of family law, immigration, and <br />healthcare advocacy. <br />Delhi Center in <br />collaboration with <br />UCI <br />Teens Engaged in <br />Learning and <br />Leadership <br />$62,053 <br />A leadership development program that trains teens to take an <br />active role in designing and implementing educational and team <br />building activities in their community that teach younger children <br />about gang prevention, problem solving, conflict resolution, <br />avoiding risk and making safe choices. <br />Girls Inc. of OC StrongHer <br />Together $34,860 <br />StrongHer Together provides at-risk teen girls with the tools to live <br />a safe and healthy life. The program will serve 8th – 12th grade <br />girls during the school year and the summer.
