FY 2025-26 ESG Program Funding Plan <br />Program Summaries <br /> <br /> <br />Homelessness Prevention and Rapid Rehousing <br />Rapid Rehousing <br />(short term and <br />long-term rental <br />assistance/security <br />deposits) <br />Volunteers of <br />America Los <br />Angeles <br />$57,000 <br />Volunteers of America Los Angeles (VOALA) serves people experiencing <br />homelessness in various areas throughout Orange County, including <br />Santa Ana. Target populations include: veterans, chronically homeless <br />individuals/families, families with children, survivors of domestic violence, <br />youth, elderly, families, and individuals with disabilities and/or substance <br />use disorders. Rapid Rehousing services include intake, and <br />individualized housing and service plan, and provision of case <br />management, housing navigation, and linkages to services and <br />resources. Navigation includes support in identifying access to affordable <br />units, negotiating rentals, reviewing lease agreements, and securing <br />habitable residences. In addition to navigation, VOALA provides rapid <br />rehousing recipients case management, budgeting, referrals to public <br />benefits, and overall post-move-in stabilization. <br />Rapid Rehousing <br />for <br />unaccompanied <br />homeless women <br />WISEPlace $57,000 <br />WISEPlace provides trauma-informed care and services to <br />unaccompanied women experiencing homelessness. The organization <br />provides case management, employment support, and financial <br />empowerment to women from age 18 to 65 and older. The women serve d <br />include young women leaving foster care, those who have suffered from <br />domestic violence or human trafficking, those who experienced a <br />financial catastrophe, or women recovering from drug abuse, alcohol <br />abuse or incarceration. WISEPlace’s Rapid Rehousing services include <br />case management, financial empowerment curriculum, housing search <br />support and placement, financial support, such as deposit assistance, <br />security deposits, help with moving costs and utilities. <br />Data Collection HMIS (3%) <br />HMIS – Homeless <br />Management <br />Information <br />System <br />Orange County <br />United Way $58,146 <br />Orange County United Way will facilitate HMIS coordination, <br />communication, training, and technical assistance of Santa Ana ESG <br />sub-recipients to ensure adherence to data quality, supporting the City's <br />efforts to fully comply with the HEARTH Act of 2009. Orange County <br />United Way will: provide user meetings, perform agency audits, provide <br />training/technical assistance, run data quality reports to post and make <br />available online, complete project set-ups, update the dashboard on a <br />monthly basis. <br />Total Funding $377,330 <br />EXHIBIT 5
