youth during Program Year 2008/2009 and currently serving 20 youth in Program Year <br />2009/2010 through The Foster Youth Lia ison Project. The Peer Mentors involved with <br />this program receive valuable training on participant enrollment and exit procedures, <br />client assessment, TABE testing, case recording, case management strategies and <br />community resources including services at the Santa Ana WORK Center. The Peer <br />Mentors also participate in four trainings per year which focus on topics such as: <br />• Program Values • Working with Adolescents <br />• Effective Listening & • Leadership <br />Communication Skills • Counter-Transference <br />• Boundaries • Identifying & Dealing with <br />• Problem Solving & Conflict Feelings <br />Resolution • Empathy <br />• Dealing with Emotions • 8 Commandments of Peer <br />• Group Facilitation Mentoring <br />• Team Building • Social & Business Etiquette <br />• Public Speaking • Understanding Child Abuse <br />• Resources for Youth & Neglect <br />• Assertiveness • Scenarios & Role Play <br />3 FYLP Peer Mentors who participated in the 2009/2010 program year will return for <br />2010/2011, which means trained, and experienced Peer Mentors will be involved from <br />the beginning of the new program year. <br />4. List the positions dedicated to this proposal. Include a description of duties and time <br />allocation for each responsibility. <br />4 Peer Mentors @ .450 FTE each (Peer Mentor positions are P/T 18 hours per week) <br />Job Duties Include: <br />• Recruit participants and develop My Action Plan (MAP) for each participant <br />• Intensive case management with a 6:1 peer mentor to participant ratio <br />• Case recording in on-site client files <br />• Be available 18 hours per week in the Orangewood Resource Center <br />• Provide pre-employment services and supportive services <br />• Develop an individualized tutoring program for those participants who are assessed to <br />be basic skills deficient <br />• Provide Post Assessment upon program exit and 12 months of follow-up services <br />1 Foster Youth Liaison Proiect Supervisor @ .50 FTE <br />Job Duties Include: <br />• Participate in Santa Ana Youth Council Activities (i.e. Youth Council meetings, <br />Youth Service Provider Network (YSPN) meetings, etc.) <br />• Plan and facilitate a monthly communication meeting with Peer Mentors (invitation <br />will include staff from the City of Santa Ana Workforce Investment Board) <br />• Meet with individual Peer Mentors on a regular basis to provide supervision and <br />support <br />• Be available to provide support and resources to the FYLP Peer Mentors and program <br />participants <br />• Assist City of Santa Ana staff during program audits <br />• Ensure contract compliance <br />OCF/Foster Youth Liaison Project PY 10/11 18 <br />EXHIBIT A
