YOUTH ELIGIBILITY CRITERIA [Notice PIH 2023-04; FYI Q&As; FYI FAQs] <br />The PCWA is responsible for certifying that the youth has prior qualifying foster care <br />involvement. As determined by the PCWA, eligible youth: <br /> Are at least 18 years of age and not more than 24 years of age (have not yet reached their <br />25th birthday); <br />- Youth must be no more than 24 years of age at the time the PCWA certifies them as <br />eligible and at the time of HAP contract execution. <br /> Have left foster care or will leave foster care within 90 days, in accordance with a transition <br />plan described in section 475(5)(H) of the Social Security Act; and <br />- Placements can include, but are not limited to, placements in foster family homes, foster <br />homes of relatives, group homes, emergency shelters, residential facilities, childcare <br />institutions, and pre-adoptive homes in accordance with 24 CFR 5.576; <br /> Are homeless or at risk of becoming homeless at age 16 and older. <br />- At risk of being homeless is fully defined at 24 CFR 576.2. <br />o This includes a person that is exiting a publicly funded institution, or system of care <br />(such as a healthcare facility, a mental health facility, foster care or other youth <br />facility, or correction program or institution). Therefore, youth being discharged from <br />an institution may be eligible for an FYI voucher [FYI FAQs]. <br />Eligibility is not limited to single persons. For example, pregnant and/or parenting youth are <br />eligible to receive assistance assuming they otherwise meet eligibility requirements. PHAs may <br />not exclude pregnant or parenting youth from service. <br />EXHIBIT 1
