EXHIBIT 1 <br />7-I.G. LEVEL 3 VERIFICATION: WRITTEN, THIRD -PARTY FORM <br />[Notice PIH 2O23 -271 <br />This type of verification is a form developed by the PHA and used uniformly for all families <br />when needed to collect information from a third -party source. This is known as "traditional third - <br />party verification." PHAs send a PHA -developed form directly to the third -party source by mail, <br />fax, or email and the source completes the form by hand (in writing or typeset). <br />The PHA may use this method when higher forms are unavailable or are rejected by the PHA or <br />when the family is unable to provide acceptable verification. The PHA may skip this level of <br />verification and may instead substitute oral third -party verification before moving to self - <br />certification. <br />SAHA Policy <br />Typically, SAHA will attempt to send written third -party verification forms to the <br />verification source whenever higher forms of verification are unavailable. <br />However, on a case -by -case basis, SAHA may choose to obtain oral third -party <br />verification without first attempting, and in lieu of, a written -third party verification form. <br />7.I.H. LEVEL 2: ORAL THIRD -PARTY VERIFICATION [Notice PIH 2O23-271 <br />For third -party oral verification, PHAs contact sources, identified by UIV techniques or by the <br />family, by telephone or in person. <br />Third -party oral verification may be used when requests for written third -party verification forms <br />have not been returned within a reasonable timee.g., 10 business days. <br />PHAs must document in the file the date and time of the telephone call or visit, the name of the <br />person contacted, the telephone number, as well as the information confirmed. <br />The PHA may skip this level of verification if they attempted written third -party verification via <br />a form and the source did not respond and move directly to self -certification. <br />SAHA Policy <br />In general, SAHA will attempt to obtain written third -party verification via a form from <br />the verification source. If written third -party verification forms are not returned within 14 <br />calendar days, SAHA will accept self -certification from the family without attempting to <br />obtain oral third -party verification. <br />However, if SAHA chooses to obtain oral third -party verification, SAHA will document <br />in the file the date and time of the telephone call or visit, the name of the person <br />contacted and the telephone number, as well as the information confirmed. <br />025 <br />City Council — 384 5 2 25 <br />
