LEVEL 1: NON -THIRD -PARTY VERIFICATION: SELF -CERTIFICATION <br />[Notice PIH 2O23-27] <br />Non -third -party verification consists of a signed statement of reported income and/or expenses. <br />This verification method should be used as a last resort when the PHA has not been successful in <br />obtaining information via all other required verification techniques. <br />Self -certification, however, is an acceptable form of verification when: <br />• A source of income is fully excluded <br />• Net family assets are less than or equal to the HUD -published threshold ($50,000 for 2024, <br />and $51,600 for 2025) and the PHA has adopted a policy to accept self certification <br />• The family declares that they do not have any present ownership in any real property <br />• A family reports zero income; <br />• A family states that they have non -recurring income that will not be repeated in the coming <br />year; and/or <br />• The PHA has adopted a policy to implement streamlined verification for fixed sources of <br />income (See Chapter 11) <br />When the PHA was required to obtain third -party verification but instead relies on self - <br />certification, the family's file must be documented to explain why third -party verification was <br />not available. <br />HUD does not require that a self -certification be notarized; however, HUD recommends <br />including language on any self -certification to ensure the certifier understands the consequences <br />of knowingly providing false information. <br />SAHA Polic <br />When information cannot be verified by a third party or by review of documents, family <br />members will be required to submit self -certifications attesting to the accuracy of the <br />information they have provided to SAHA. <br />SAHA may require a family to certify that a family member does not receive a particular <br />type of income or benefit. <br />The self -certification must be made in a format acceptable to SAHA and must be signed <br />by the family member whose information or status is being verified. SAHA will not <br />adopt self -certification of assets. <br />All self -certifications will include the following language: <br />"I/We, the undersigned, certify under penalty of perjury that the information provided <br />here is true and correct, to the best of my knowledge and recollection. WARNING: <br />Anyone who knowingly submits a false claim or knowingly makes a false statement is <br />subject to criminal and/or civil penalties, including confinement for up to five years, <br />fines, and civil and administrative penalties (18 U.S.C. 287, 1001, 1010, 1012; 31 U.S.C. <br />3279, 3802)." <br />025 <br />City Council — 386 7M5 2 25 <br />
