Administrative Plan 7/1/2025 <br /> <br />Page 7-1 <br />Chapter 7.A. <br />VERIFICATION <br />[24 CFR 982.516, 24 CFR 982.551, 24 CFR 5.230, Notice PIH 2018-18] <br />INTRODUCTION <br />Prior to the PHA’s HOTMA compliance date, the PHA will follow policies as outlined in this <br />chapter. Upon the PHA’s HOTMA compliance date, the PHA will follow policies as outlined in <br />Chapter 7.B. <br />The PHA must verify all information that is used to establish the family’s eligibility and level of <br />assistance and is required to obtain written authorization from the family in order to collect the <br />information. Applicants and program participants must cooperate with the verification process as <br />a condition of receiving assistance. The PHA must not pass on the cost of verification to the <br />family. <br />The PHA will follow the verification guidance provided by HUD in Notice PIH 2018-18 and any <br />subsequent guidance issued by HUD. This chapter summarizes those requirements and provides <br />supplementary PHA policies. <br />Part I describes the general verification process. Part II provides more detailed requirements <br />related to family information. Part III provides information on income and assets, and Part IV <br />covers mandatory deductions. <br />Verification policies, rules and procedures will be modified as needed to accommodate persons <br />with disabilities. All information obtained through the verification process will be handled in <br />accordance with the records management policies of the PHA. <br />PART I: GENERAL VERIFICATION REQUIREMENTS <br />7-I.A. FAMILY CONSENT TO RELEASE OF INFORMATION [24 CFR 982.516; <br />24 CFR 982.551; 24 CFR 5.230; and Notice PIH 2023-27] <br />Consent Forms <br />The family must supply any information that the PHA or HUD determines is necessary to the <br />administration of the program and must consent to PHA verification of that information [24 CFR <br />982.551]. All adult family members must sign consent forms as needed to collect information <br />relevant to the family’s eligibility and level of assistance. While PHAs must use form HUD- <br />9886-A, this form does not release all the information necessary to the administration of the <br />program. The PHA must also develop its own release forms to cover all other necessary <br />information. <br />EXHIBIT 1
