VERIFICATION OF LEGAL IDENTITY <br />SAHA Policy <br />SAHA will require families to furnish verification of legal identity for each household <br />member. <br />Verification of Legal Identity for Adults Verification of Legal Identity for <br />Children <br />Certificate of birth, naturalization papers <br />Church issued baptismal certificate <br />Current, valid driver's license or <br />Department of Motor Vehicles <br />identification card <br />U.S. military discharge (DD 214) <br />Current U.S. passport <br />Current government employer <br />identification card with picture <br />Certificate of birth <br />Adoption papers <br />Custody agreement <br />Health and Human Services ID <br />Certified school records <br />If a document submitted by a family is illegible for any reason or otherwise questionable, <br />more than one of these documents may be required. <br />If none of these documents can be provided and at SAHA’s discretion, a third party who <br />knows the person may attest to the person’s identity. The certification must be provided <br />in a format acceptable to SAHA and must be signed by the family member whose <br />information or status is being verified. <br />Legal identity will be verified for all applicants at the time of eligibility determination <br />and in cases where SAHA has reason to doubt the identity of a person representing <br />themselves to be a participant. <br />EXHIBIT 1
