Completion of class materials and assignments (evidenced by pre- and post-test <br /> scores) <br /> d. Successful completion of work-based learning assignments <br /> 5. Attendance and punctuality must be documented through signed timesheets. <br /> 6. Participation and completion of class materials must be verified by the workshop <br /> facilitator. <br /> 7. Completion of work-based learning must be documented by the worksite supervisor and <br /> career planner through: <br /> a. Signed timesheets <br /> b. Monthly evaluations <br /> c. Skill progression reports <br /> 8. Participants who have received a strike(s) per participant policy may receive an incentive <br /> at management's discretion. <br /> D. INDUSTRY-RECOGNIZED CREDENTIAL COMPLETION <br /> 1. Incentive available for youth who successfully complete a WIOA-approved training <br /> program leading to an industry-recognized credential. <br /> 2. Participants may receive incentive payments for each completed milestone up to a <br /> combined total of$500. Milestone completion must be verified through documentation. <br /> 3. Maximum incentive: $500 for successful completion, including all required exams. <br /> 4. If a participant completes both work-based learning and an industry-recognized <br /> credential, the maximum combined incentive is $500. <br /> Milestone Incentive Amount <br /> Enrolled in a WIOA-approved training program $50 <br /> Completed 50% of the training program $100 <br /> Successfully completed the training program $150 <br />
