The student-applicant will be required to submit documentation substar tiating rehabilitation, counseling, and/or retribution. If the student-applicant is found ineligible to participate in clinical courses, he/she will be required to withdraw rrom the program and will be financially responsible for all charges incurred per withdrawe 1 policy. The AL dent-applicant is responsible for the cost of the background check. It is the student's responsibility to notify the Program Director if any infractions had occurred during Lhe program. A repeat background check may be required from the student wh le in the pro gram. If the background check is unfavorable, made at discretion of the Program D irectc r, the student is subject to the dismissal policy. The background check cannot e used to replace the Live Scan requirement to apply for the CNA Exam. Eligibility for Licensure The Ca ifornia State Certified Nursing Assistant exam requires a student to have a State I or Driver icense and a social security number or taxpayer identification number to test fort e Certification exam. To take the Certified Nursing Assistant Exam, the student must complete the California Department of Public Health Form 283B Certified Nurse Assistant Initial Appl cation Information form and 60 theory hours and 100 clinical. The form must be turned into the MPH. A testi g fee of$130.00 is paid to the testing company for a skills and knowledge Exam. GL College does not pay for retake exams. NOTICE CONCERNING TRANSFERABILITY OF CREDITS AND CREDENTIALS EARNED AT OUR INSTITUTION The transferability of credits you earn at LGL College is at the complete discretion of an institut on to which you may seek to transfer.Acceptance of the Nursing Assistant Cc] ificate you cai n in Nursing Assistant program is also at the complete discretion of the institi ition to which you may seek to transfer. If the Nursing Assistant Certificate that you earn at this institut on are not accepted at the institution to which you seek to transfer,you may be r quired to repe t some or all of your coursework at that institution.
