Both parents will be provided with classes, education, <br /> and support that will teach them coping skills, resiliency,and strategies for self-care to improve their mental <br /> health.They will learn how to recognize the signs and symptoms of anxiety, depression,and other perinatal mood <br /> and anxiety disorders,and what to do if recognized. Mothers and fathers will build networks of support and <br /> relationships with other moms and dads. Pregnant women and new mothers who are survivors/victims of <br /> domestic violence will be provided with supplemental/specialized home visitation, referrals,support, and <br /> coaching. <br /> Measurable Outcomes: <br /> During FY26, MOMS'will accomplish the following: <br /> -At least 98 Santa Ana mothers,fathers, and babies each year will receive individualized education,support, and/or <br /> mental health therapy. <br /> -Less than 15%of women will give birth prematurely. <br /> -Less than 15%of babies will be horn with a low-birth weight. <br /> -Less than 15%of babies will be admitted to the NICU. <br /> -At least 90%of babies will be on-track to meet important developmental milestones at graduation. <br /> -Low-income families will receive health education and promotion of health care and vaccinations.At program exit, <br /> at least 90%of babies will have a medical home and at least 90%will be on-track with their vaccinations. <br /> -MOMS Orange County will provide approximately 3,000 home visitation encounters for residents of Santa Ana in <br /> total. <br /> -Low-income families will receive referrals to community resources that may help them meet their physical and <br /> mental health needs, and access basic needs like food, housing, domestic violence support and shelter,and <br /> financial assistance. <br /> -Low-income families will receive donations of material goods including diapers, wipes,clothing,toys, books,and <br /> even car seats as available. <br /> EXHIBIT A <br />
