Payee <br /> <br />Name: City of Santa Ana Amount: $ 5,878,393.38 <br /> <br />2. Budget Detail & Changes <br /> <br />A. The Grantee agrees that HHAP-5 funds shall be expended on uses that <br />support regional coordination and expand or develop local capacity to <br />address immediate homelessness challenges. Such activities must <br />be informed by a best-practices framework focused on moving people <br />experiencing homelessness into permanent housing and supporting the <br />efforts of those individuals and families to maintain their permanent <br />housing. <br /> <br />B. The Grantee shall expend the HHAP-5 funds on eligible activities <br />as detailed in HSC 50234, subdivision (a)(1)(A) and 50236 and as <br />described in the grantees latest approved HHAP-5 funding plan. <br /> <br />C. Any changes to the Grantee’s budget must be approved in writing by HCD <br />prior to incurring expenses. <br />                  <br />3. General Conditions Prior to Disbursement  <br />  <br />A. All Grantees must submit the following forms prior to this HHAP-5 <br />allocation being released:  <br />  <br />1) Request for Funds Form (“RFF”).  <br /> <br />2) STD 213 Standard Agreement form and Exhibits A through E. <br /> <br />3) STD 204 Payee Data Record or Government Agency Taxpayer ID <br />Form. <br /> <br />4.
