Agreement Statement <br />Proposal shall include a statement outlining your concurrence or reference to concerns <br />previously submitted with any and all provisions as contained in EXHIBIT II – Sample <br />Agreement of this RFP (if any). <br />d. Firm and Team Experience <br /> Proposal shall include a profile of the firm’s experience including the following: <br />i. A general description of the firm, including size and number of employees <br />working directly with the City on this agreement. <br />ii. Firm’s nearest address serving the City of Santa Ana and headquarters <br />address. <br />iii. Name and contact information of the supervising Project Manager/Principal <br />Agent, to be assigned to the agreement. The Project Manager/Principal <br />Agent shall be the primary contact person to represent your firm and will be <br />the person to conduct the presentation, if invited to an interview. <br />iv. Resumes for all key staff proposed describing relevant experience. Identify <br />personnel who will work on the monitoring including their resumes and <br />hourly rates. <br /> <br />v. Describe recent monitoring experience similar to the type requested. <br /> <br />e. Proposed Work Plan <br />Proposal shall include a statement demonstrating the firm’s understanding of the <br />Scope of Services. <br /> <br />Describe the company’s experience and work plan to conduct financial and <br />programmatic compliance reviews of federal grants with your hourly rates and <br />estimated hours for the following services: <br /> <br />• Financial monitoring of up to five (5) Community Development Block Grant <br />sub-recipients providing fair housing and/or public services. <br /> <br />• Program monitoring for up to five (5) Community Development Block Grant <br />sub-recipients providing fair housing and/or public services. <br /> <br />• Financial monitoring of up to three (3) Emergency Solutions Grant sub- <br />recipients. Two sub-recipients provide one activity and one subrecipient <br />provides three activities. <br /> <br />• Project monitoring of two (2) Community Development Block Grant capital <br />improvement projects. <br /> <br />• Financial monitoring of two (2) HOME projects. <br /> <br />• Technical support to enhance project documents and reporting documents for <br />City of Santa Ana RFP No. 25-076A Page 7 of 36 <br />EXHIBIT 1
