Total number of unduplicated clients (Santa Ana and Non-Santa Ana Residents) anticipated to be served by the <br /> funded program, named above, during the 12-month contract period. <br /> 20 Persons <br /> II. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12-month <br /> co--n--trraactpe�eriod. <br /> L6Persons <br /> Program and Funding Description <br /> III. Description of Work- In the space below, describe the program to be funded during the 12-month contract period. <br /> What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br /> viewable space will print. <br /> During the contract period, Covenant House California (CHC)will operate a 20-bed low-barrier emergency shelter <br /> in Anaheim for youth ages 15-24 experiencing homelessness.CHC will provide 24/7 access to shelter and <br /> intensive wraparound services including individualized case management,crisis intervention, resource <br /> coordination, meals, hygiene facilities,and access to medical and mental health care. <br /> Youth will undergo an intake process followed by a Social and Emotional Wellbeing(SEWB) assessment and the <br /> development of a customized Success Plan to guide theirjourney toward housing stability. Weekly case <br /> management sessions will support housing navigation,connection to benefits (e.g., Medi-Cal, CalFresh), <br /> education and employment pathways,family reunification (when safe),and access to legal aid and behavioral <br /> health services. <br /> The average length of stay is 127 days, and no fees or program prerequisites are required. <br /> The funding received be allocated to one position,a youth engagement specialist,who works in the shelter site <br /> and provides support and resources for 6 Santa Ana young people who are experiencing homelessness. <br /> Schedule of Performance <br /> Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12-month <br /> contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
