Participants will also learn advocacy skills during one-on-one case <br /> management,support groups, and counseling sessions.The Case Manager will serve as a Victim/Witness Liaison <br /> and peer support during medical screenings and criminal or court proceedings. Case Managers will also serve as <br /> community advocates to address all immediate needs. <br /> 8.Comprehensive Supportive Services: Provide assistance in benefit entitlement enrollment(e.g.California <br /> Victims Compensation Program,SSI/SSDI, Medicaid); education services, locating childcare programs, parenting <br /> courses, health literacy, housing support, and linkage to healthcare and other mainstream service providers. <br /> 9. Follow-up and Evaluation:The Case Manager and Counselor will continue to monitor and evaluate participants' <br /> progress toward achieving self-sufficiency through the completion of milestones in case management or <br /> counseling sessions on a weekly basis.VIP is structured as a walk-in daytime drop-in center to increase community <br /> access to victim services. Hours of operations include Monday through Friday from 9:00 am— 12:00 pm and 1:00 <br /> pm—3:00 pm.VIP participants are enrolled in a three month intense case management track with weekly one-on- <br /> one touchpoints through both in-person appointments or via phone. Behavioral health counseling and group <br /> sessions are delivered biweekly for three months through both in-person, <br /> online,or telehealth delivery methods. Emergency assistance is provided on a daily basis;financial assistance (rent, <br /> motel, utilities,transportation,etc.) is delivered on a monthly basis and limited to a one-time participation per 12 <br /> months.Additional service frequency includes Outreach—Daily;Screening and Needs Assessment—Daily; <br /> Advocacy—Daily/As needed;Supportive Services—Daily/As needed; Life Skills Training—Weekly;and Job <br /> Readiness—Weekly/As needed.The target population is women and moms with children who have been a victim <br /> of Domestic Violence/Intimate Partner Violence (DV/IPV) in Santa Ana.These crimes include primarily domestic <br /> violence and other crimes that fall under the intimate partner abuse category(rape, sexual assault,sexual battery, <br /> assault, and battery).Other characteristics of victims served include homelessness, mental impairment(other co- <br /> occurring disorders),and unemployment. LSSSC aims to allocate the requested funding to serve 60 Santa Ana- <br /> based participants annually.All VIP participants are verified as living below 150%of the federal poverty level. <br /> Additional participant demographics include: <br /> 49%Hispanic/Latino <br /> 28%White <br /> 19%Asian <br /> 2%Black/African American <br /> 2%Other <br /> EXHIBIT A <br />
