4. CERTIFICATIONS <br />The following forms, included in ATTACHMENT 5: CERTIFICATIONS in the <br />Appendix of this RFP, shall be signed and included as part of the proposal submittal <br />package: <br />• Non -Collusion Affidavit <br />• Non -Lobbying Certification <br />• Non -Discrimination Certification <br />IV. PROPOSAL REVIEW CONSULTANT/CONTRACTOR SELECTION <br />A. EVALUATION AND RATING <br />Refer to SECTION I "GENERAL" above. <br />B. SELECTION <br />The committee may interview the top rankinq proposers. The City will recommend <br />award of the agreement to the Proposer who will provide the best quality and value to <br />the City. City reserves the right to begin negotiations and enter into an agreement <br />without interview or further discussions. <br />V. AWARD OF AGREEMENT <br />A. REQUEST FOR COUNCIL ACTION <br />Following evaluation and rating by the proposal review committee, the Executive <br />Director will recommend award of an agreement to the proposer providing the best <br />quality and value to the City. <br />B. EXECUTION OF AGREEMENT <br />A standard agreement is included as ATTACHMENT 4: STANDARD AGREEMENT in <br />the Appendix of this RFP. "Proposer" will hereinafter be referred to as "Consultant" in <br />standard agreement. The term of the agreement will begin after the agreement is fully <br />executed, and all required bonds, insurance documents and contents of the payment <br />information packet have been received and approved. <br />VI. IMPLEMENTATION <br />A. KICK-OFF MEETING <br />A kick-off meeting will be held after award of contract. Consultant and its team will meet <br />with City of Santa Ana staff to conduct introductions, discuss scope of services, and <br />implementation process. <br />B. NOTICE TO PROCEED <br />Following the kick-off meeting, a formal Notice to Proceed (NTP) may be issued after <br />City of Santa Ana RFP 25-046 <br />Programmable Logic Controller (PLC) Programming and Consulting Services <br />Page 12 <br />
