Reports not received in CDFS by the 20th of the month, following the end of <br />the contractor’s reporting period, shall be deemed delinquent and <br />apportionment(s) shall be withheld until the required report is received. <br />3. Contractors shall submit reports containing the following information for each <br />contract to the CDFS: <br />a. Total days of operation in the current reporting period and year to date; <br />b. Amount and sources of all revenues, other than advanced contract funds, <br />for the current reporting period and the year-to-date total; <br />c. Total expenditures related to the program operation for the current <br />reporting period and the year-to-date total. <br />4. Contractors will submit and certify reports online by staff authorized to confirm <br />that the information contained in the report is correct and complete. <br />5. Contractors have sixty (60) days from the due date for submission of the audit <br />to submit a revised final fiscal report. For local educational agencies, the final <br />report shall be the final accounting of any amount payable to or receivable <br />from the contractor pursuant to this contract. <br />L. Service Data Report for Resource and Referral Programs (5 CCR 18069) <br />CRRP contractors shall submit reports to the CDSS which contain the following <br />data on a quarterly basis. Penalties for delinquent reporting are specified. <br />1. Number of requests for general childcare information and childcare referrals; <br />2. Age categories of childcare requests and referrals: <br />a. Infant (birth to eighteen months); <br />b. Toddlers (eighteen months to thirty-six months); <br />c. Preschool (three years to kindergarten enrollment; and <br />d. School age (kindergarten enrollment to age 13). <br />3. Time categories of childcare referrals: <br />a. Full-time; <br />b. Part-time. <br />4. Number of children needing: <br />a. Before and/or after school;
