qualification. In addition to the above, the City Manager may grant <br /> a further extension not to exceed a total of one (1) year without <br /> pay. <br /> F. Extension by Use of Vacation. After an employee' s <br /> sick leave has been exhausted he or she may be granted permission <br /> to take any earned vacation he or she may have accrued. <br /> G. Notice . The employee taking sick leave shall notify <br /> his or her immediate supervisor not less than one (1) hour prior to <br /> the time he or she is scheduled to report for duty. When the <br /> absence is more than three (3) consecutive working days, the <br /> employee must present to his or her department head a physician' s <br /> certificate stating the cause of illness and that, in the <br /> physician' s opinion, the employee could not report to work because <br /> of such illness or injury and that the employee is sufficiently <br /> recovered to safely return to work. Such certificate shall be <br /> transmitted to the Chief Personnel Officer with the report of the <br /> return of the employee to work. <br /> A physician' s certificate or other satisfactory written <br /> evidence of actual illness or injury may be required after an <br /> absence of any duration of less than three (3) days . <br /> H. Denial . No employee shall be entitled to sick leave <br /> with pay while absent from duty because of sickness or injury <br /> purposely self-inflicted or caused by willful misconduct; or <br /> sickness or disability sustained while engaged in employment other <br /> than employment by the City, for monetary gain or other <br /> compensation, or by reason of engaging in business or activity for <br /> monetary gain or other compensation. <br /> I . Excess Usage . If sick leave is used in excess of that <br /> due and available to an employee, such excess sick leave, will <br /> first, be deducted from any available compensatory time off <br /> benefit; second, from any available vacation leave benefit; <br /> finally, deducted from the next scheduled wage or salary payment . <br /> J. Personal Necessity Leave . Each employee shall be <br /> afforded the opportunity to use up to three (3) days (24 hours) of <br /> sick leave benefits per calendar year (on a non-cumulative basis) <br /> for "personal necessity leave, " which circumstances shall include <br /> serious accidents or illnesses to members of the employee' s <br /> immediate family, childbirth, imminent danger to the employee' s <br /> home or other valuable property, or the existence of external <br /> circumstances beyond the employee' s control making it impractical <br /> for him/her to report for duty. <br /> K. Payment for Unused Sick Leave. <br /> 1 . Except in cases of disability retirement, upon <br /> non-disciplinary termination of employment after ten (10) years of <br /> 29 <br />
