STATEWIDE SANITARY SEWER SYSTEMS GENERAL ORDER 2022-0103-DWQ <br />• Updated sewer system service area boundary map, if service area boundary has <br />changed from original map submitted per section 5.14 (Electronic Sanitary Sewer <br />System Service Area Boundary Map) of this General Order; <br />• Number of system operation and maintenance staff: <br />o Entry level (less than two years of experience), <br />o Journey level (greater than two years of experience), <br />o Supervisory level, and <br />o Managerial level; <br />• Number of operation and maintenance staff certified as a certified collection system <br />operator by the California Water Environmental Association (CWEA), with: <br />o Corresponding number of certified collection system operator grade levels <br />(Grade I, II, III, IV, and V); <br />• System information: <br />o Miles of system gravity and force mains, <br />o Number of upper and lower service laterals connected to system, <br />o Estimated number of upper and lower laterals owned and/or operated by the <br />Enrollee, <br />o Portion of laterals that is Enrollee's responsibility, <br />o Average age the major components of system infrastructure, <br />o Number and age of pump stations, and <br />o Estimated total miles of the system pipeline not accessible for maintenance; <br />• Name and location of the treatment plant(s) receiving sanitary sewer system's <br />waste; <br />• Name of satellite sewer system tributaries; <br />• Number of system's gravity sewer above or underground crossings of water bodies <br />throughout system; <br />• Number of force main (pressurized pipe) above or underground crossings of water <br />bodies throughout system; <br />• Number of siphons used to convey waste throughout the sewer system; <br />• Miles of sewer system cleaned; <br />• Miles of sewer system video inspected, or comparable (i.e., video closed-circuit <br />television or alternative inspection methods); <br />• System Performance Evaluation as specified in section 5.11 (System Performance <br />Analysis) of this General Order; <br />• Major spill causes (for example, root intrusion, grease deposition); <br />ATTACHMENT E1 — NOTIFICATION, MONITORING, REPORTING <br />AND RECORDKEEPING REQUIREMENTS December 6, 2022 <br />E1 - 18 <br />
