EXHIBIT A <br /> SCOPE OF WORK <br /> Purpose: City of Santa Ana (City)shall provide all labor, materials, tools,equipment, and supervision <br /> necessary to provide as-needed maintenance and repair services to Change County Fire Authority <br /> (OCFA) Heavy fluty and tight Duty vehicles and apparatus. The worn In general is to trouble shoot, <br /> diagnose, tune-up,service and repair Heavy Duty and Light Duty vehicles and apparatus(services). <br /> Services shall be provided on an as-needed basis as required by the OCFA Fleet Services department, <br /> Scope of Work: On as as-needed,as requested basis, City shall perform preventative maintenance(PM) <br /> services that include, but are not limited to. PM levels A&E, for heavy and light duty vehicles and <br /> apparatus. OCFA Fleet Services to provide PM check list(See Exhibits C-F).As-needed miscellaneous <br /> diagnosis and repairs may Include, but are not limited to,worts on, discharge valves,electrical,charging <br /> system,oil leaks,coolant system,air system,tire repair, seat belt alarm,open compartment warning, <br /> engine performance, brake system,and other repairs normal and customary for routine repair of heavy <br /> and light duty vehicles and apparatus. <br /> Work shall not be sub-contracted without approval from the OCFA Fleet Services designated <br /> representative,Any approved subcontractor utilized for maintenance or repair work must be able to meet <br /> the OCFA's required qualifications,repair certificates, licenses,and insurance requirements. <br /> Service Requirements: <br /> a. City's facility shall comply with all Federal,State, and Local laws and regulations. <br /> b. Authorized representatives of the OCFA shall be permitted to inspect the facility on poriodic basis <br /> with advance notice to the City. More frequent inspectlons shall be permitted with advance notice <br /> to the City if problems develop and remeln unresolved, Persistent and/or unresolved performance <br /> or other Issuers may be grounds for termination of his agreement. <br /> c. All services provided under this Agreement shall comply with all applicable Federal,State,and <br /> Local regulations. <br /> d, Prior to the commencement of any work or repairs on OCFA equipment, City shall provide a <br /> written estimate to the OCFA Fleet Services authorized representative(Fleet Services Manager <br /> or Fleet Services Supervisor)for the requested or re�commendad repairs.Approval from a Fleet <br /> Services authorized representative must be obta€riad prior to worts being performed and shall be <br /> based on the provided estimated cost. If additional work Is required, approval from the OCFA <br /> Fleet Services designated representative must be obtained prior to work being completed. <br /> e. All items provided under this Agreement shall be new and origlnai equipment manufacturer <br /> (OEM)line of replacement parts/components or approved aftermarkut parts.All items provided <br /> under this Agreements shall be equivalent in function and workmanship to the parts originally <br /> supplied on the vehicle(s)unless superseded by the vehicle manufacture.Reba€It remainufaetured <br /> or other deviations or substitutions will not be allowed without advance written or verbal approval <br /> by the OCFA Fleet Services department. NUTS:QCFA reserves the right to supply any parts <br /> required to perform repairs. In the event that is done City will not warranty any parts provided by <br /> OCFA. <br /> f. All worst shall be inspected by an authorized OFCA Fleet Set-vices representative for proper <br /> completion and correction of the reported concerns. If it is determined that the concerns were not <br /> properly addressed or the quality of work is unacceptabile,QCFA will contact the City and provide <br /> an explanation of what work was unacceptable. If City agrees the work was unacceptable, City <br /> will re-do work at City`s expense. If City does not agree that the wor'K performed was <br /> unacceptable,the Parties will negotiate a resolution that is mutually agreeable to the parties, <br />
