CITY OF SANTA ANA <br />ATT ACHMENT G <br />SUBCONTRACTOR DESIGNATION FORM <br />Proposer acknowledges and agrees that under Public Contract Code section 4100, et seq., it must clearly <br />set forth below the name and location of each subcontractor who will perform work or labor or render <br />SerViCe tO the proposer in Or aboutthe WOrk in an amount in eXCeSS Of one-half Of One percent (0.5%) Of <br />proposer's total bid and the kind of work that each will perform. Furthermore, proposer acknowledges <br />and agrees that under Public Contract Code section 4100, et seq., if proposer fails to list as to any portion <br />of work, or if proposer lists more than one subcontractorto perform the same portion ofwork (i.e. proposer <br />must indicate what portion of the work each subcontractor will perform), proposer must perform that <br />portion itself or be subjected to penalty under applicable law. <br />If altemate bids are called for and proposer intends to use subcontractors different from or in addition to <br />those subcontractors listed for work under the base bid, proposer must list subcontractors that will <br />perform work in an amount in excess of one half of one percent (0.5%) of proposer's total bid, including <br />alternates. <br />In case more than one subcontractor is named for the Same kind of work, the Contractor is to state the <br />portion of work that each subcontractor will perform. Proposers or suppliers of materials only do not need <br />to be listed. If further space is required for the list of proposed subcontractors, additional sheets showing <br />the required information, as indicated below, shall be attached hereto and made a part of this document. <br />Listed below is the name of each subcontractor that will perform work, labor, or render services to the <br />undersigned related to the work of this project.
