Total number of unduplicated clients(Santa Ana and Non-Santa Ana Residents)anticipated to be served by the <br /> funded prc ram, named above, during the 12-month contract period. <br /> 100 Persons <br /> Il. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12-month <br /> contract period. <br /> 50 Persons <br /> Program and Funding Description <br /> III. Description of Work-In the space below, describe the program to be funded during the 12-month contract period. <br /> What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br /> viewable space will print. <br /> Santa Ana Police Officers will contact individuals experiencing homelessness to provide the person(s)with <br /> necessary resources to help end their cycle of homelessness.The various resources will include lodging atthe <br /> Santa Ana Navigation Center,Yale Navigation Center,or other available homeless shelters and housing <br /> programs.HEART trained officers will also provide transportation services and connect individuals with mental <br /> health services along with the Orange County Psychiatric Emergency and Response Team(PERT)team <br /> members. HEART trained officers will also provide individuals with connections to medical services and drug <br /> rehabilitation programs.Through a collaborative and dedicated approach,officers will work throughout the <br /> entire City,focusing on areas regularly impacted by persons experiencing homelessness,whether reported <br /> though calls for service,citizen complaints,city officials,or visibly noticeable encampments. <br /> The full amount of the grant award will be expended as overtime with a minimum and maximum range for the <br /> overtime rates based on position title.The H.E.A.R.T.program will utilize overtime staffing as an operational <br /> necessity to avoid disrupting core patrol functions while maximizing the effectiveness of outreach efforts <br /> during those specific hours.Officers participating in the H.E.A.R.T.Program will do so voluntarily during off- <br /> duty hours,allowing for greater flexibility in contacting individuals experiencing homelessness during early <br /> mornings,late evenings,and weekends;timeframes that are critical for successful engagement,as recognized <br /> in 24 CFR§576.101. The payroll expenses that will be charged to the ESG grant will reflect overtime hours. <br /> Officers will also conduct substantial outreach efforts during regular duty hours(straight time),which will be <br /> absorbed by the Santa Ana Police Department(SAPD)and not billed to the ESG grant.This dual approach will <br /> ensure continual engagement with the homeless population without overburdening city resources or <br /> compromising public safety coverage. The SAPID will maintain timekeeping records,engagement logs,and <br /> required documentation.Furthermore,officer activity and performance outcomes will be tracked and <br /> reported. <br /> Schedule of Performance <br /> Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12-month <br /> contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
