(9) <br />described in EXHIBIT I — Scope of Services. <br />c. Agreement Statement <br />Proposal shall include a statement outlining your concurrence or reference to concerns <br />previously submitted with any and all provisions as contained in EXHIBIT II — Sample <br />Agreement of this RFP (if any). <br />d. Firm and Team Experience <br />Proposal shall include a profile of the firm's experience including the following: <br />A general description of the firm, including size and number of employees <br />working directly with the City on this agreement. <br />Firm's nearest address serving the City of Santa Ana and headquarters <br />address. <br />iii. Name and contact information of the supervising Project Manager/Principal <br />Agent, to be assigned to the agreement. The Project Manager/Principal <br />Agent shall be the primary contact person to represent your firm and will be <br />the person to conduct the presentation, if invited to an interview. <br />iv. Resumes for all key staff proposed describing relevant experience. <br />e. Proposed Work Plan/Understanding of Need <br />Proposal shall include a statement demonstrating the firm's understanding of the <br />Scope of Services. <br />Additionally, proposed work plan shall include Proposers': <br />i. Anticipated approach to performing services as specified herein; <br />ii. Suggestions or special concerns the evaluation committee should take into <br />consideration (if any); <br />iii. Description of deliverables and implementation plan. Proposer shall submit a <br />general description of the deliverables, implementation plan, and timeline. <br />f. Project Controls <br />Describe your firm's ability to control costs and provide accurate and timely <br />invoices through internal control measures; to monitor and stay within budget; <br />to monitor schedule and review times and describe the techniques used to <br />complete projects within the proposed time frames. <br />Provide a description of your firm's QA/QC Process/procedures and explain <br />how it will ensure quality work and an on -time product delivery. <br />g. CARB Fleet Compliance <br />The City is a Public Works Awarding Body, as defined under Title 13 California Code <br />of Regulations section 2449(c)(46). Accordingly, Proposers must submit, with their <br />Proposals, valid Certificates of Reported Compliance ("CRC") for the Proposer's fleet <br />and for the fleet(s) of its listed subcontractors (including any applicable leased <br />equipment or vehicles). Proposer must additionally complete and submit the Fleet <br />Compliance Certification, included herein as ATTACHMENT G. Failure to provide a <br />
