(9) <br />EXHIBIT 3 <br />4. Specify, in detail, the grounds of the protest and the facts supporting the protest; and <br />5. Include all relevant, supporting documentation with the protest at time of filing. <br />If the protest does not comply with each of these requirements, it will be rejected as invalid. If <br />the protest is valid, the City's Purchasing Manager, or other designated City staff member, <br />shall review the basis of the protest and all relevant information. The decision from the <br />Purchasing Manager, or her/her designee, is final and no further appeals will be considered. <br />VI. CERTIFICATIONS (ATTACHMENTS) <br />In addition to the SOQ, Narrative/Technical Proposal, and Cost Proposal, the following forms, <br />included in this RFP, shall be signed and included as part of the proposal submittal package: <br />• Attachment A: <br />Proposer's Certification and Proposal Item Pricing <br />Attachment B: <br />Proposer's Statement <br />Attachment C: <br />Non -Collusion Affidavit <br />Attachment D: <br />Non -Lobbying Certification <br />Attachment E: <br />Non -Discrimination Certification <br />• Attachment F: <br />Subcontractor Designation Form <br />• Attachment G: <br />CARB Fleet Compliance Certification <br />The proposal must be completely responsive to the RFP. Incomplete proposals will be deemed <br />as nonresponsive and will be rejected. The City reserves the right to reject any or all proposals <br />submitted and no representation is made hereby that any commitment will be awarded pursuant <br />to this RFP or otherwise. <br />PLEASE NOTE. <br />• All forms above must be signed by a representative of the Firm that is legally <br />authorized to contractually bind the Proposer. <br />• City will not waive notarization requirement when applicable on any of the <br />required attachments. <br />VII. REFERENCES <br />Contractor shall provide three (3) references from other similar public agencies for which services <br />similar to those specified in this RFP have been performed, including contact names and <br />telephone numbers. The respondent grants permission for the City to contact any individuals <br />listed as references. <br />City may disqualify a Proposer if - <br />References fail to substantiate Proposer's description of services and <br />deliverables provided; or <br />• References fail to support that Proposer has a continuing pattern of providing <br />capable, productive, and skilled personnel, or <br />
