3— ELECTRONIC ITEM 21/BULK SUBMISSION ATTACHMENT SHEET <br /> Respondent shall provide itemized pricing for all equipment to be included in this Proposal Response. The total price shown shall equal the amount the <br /> Respondent has provided on their submitted Proposal Form. <br /> An electronic copy will be required at the time of proposal. <br /> Files will be returned in the same format as the published,no exceptions. Failing to comply with these requirements will be considered for proposal <br /> dismissal. <br /> Respondent shall include a Description,Part Number,Quantity and Price for each relevant item included in their proposal.Respondent shall provide the <br /> Labor cost as a line item. <br /> Provided by the "Service Provider" <br /> ITEM 21 SUBMISSION REQUIREMENTS <br /> Vendors must complete and submit the provided Item 21 bulk upload spreadsheet template.
