Narrative: Narrative <br />Friday, June 25th, 2021, 1158 hours: <br />Santa Ana Police Department Homicide Detective Sergeant M. Sarabia #2565 notified me of an in-custody death that <br />RFFXUUHG DW WKH 6DQWD $QD -DLO 6HUJHDQW 6DUDELD UHTXHVWHG WKDW , UHVSRQG WR WKH 6DQWD $QD 3ROLFH 'HSDUWPHQW LQ <br />order to assist with the investigation. <br />1325 hours: <br />I responded to the Santa Ana Police Department and was provided a preliminary briefing of the incident by Sergeant <br />6DUDELD 3UHVHQW DW WKLV EULHILQJ ZHUH SHUVRQQHO IURP WKH 6DQWD $QD 3ROLFH +RPLFLGH 6HFWLRQ DQG WKH 2UDQJH <br />County District Attorney Special Assignments Unit. <br />Sergeant Sarabia informed us that today, at approximately 1125 hours, Santa Ana Jail Supervisor M. Monreal #3181 <br />contacted the Santa Ana Police Department Watch Commander to report that an inmate was down, as a result of a <br />VXLFLGH DWWHPSW 2UDQJH &RXQW\ )LUH $XWKRULW\ 0HGLFV ZHUH UHVSRQGLQJ WR WKH MDLO DQG D SDWURO RIILFHU ZDV EHLQJ <br />UHTXHVWHG WR IROORZ WKH PHGLFV ZLWK WKH LQPDWH WR WKH KRVSLWDO $W DSSUR[LPDWHO\ KRXUV WKH LQPDWH ZDV <br />pronounced deceased by medics and the inmate was not transported to the hospital. <br />7KH GHFHDVHG LQPDWH ZDV VXEVHTXHQWO\ LGHQWLILHG DV 'DUU\OH 0DUFHOO 6DPXHO 6DPXHO ZDV EHLQJ KRXVHG LQ FHOO <br />DORQH LQ 0RGXOH & 6DPXHO ZDV D IHGHUDO 86 0DUVKDOV LQPDWH ZKR ZDV DFWLQJ DV DQ LQIRUPDQW RQ D KRPLFLGH FDVH <br />1415 hours: <br />I responded to the Santa Ana Jail with additional Detectives from the Santa Ana Police Homicide Section and <br />,QYHVWLJDWRUV IURP WKH 2UDQJH &RXQW\ 'LVWULFW $WWRUQH\ 6SHFLDO $VVLJQPHQWV 8QLW :H ZHUH VXEVHTXHQWO\ HVFRUWHG WR <br />WKH WK )ORRU 0RGXOH & 6XSHUYLVRU 0RQUHDO FRQGXFWHG D EULHILQJ UHJDUGLQJ WKH NQRZQ FLUFXPVWDQFHV RI WKH <br />LQFLGHQW 3UHVHQW DW WKLV EULHILQJ ZHUH SHUVRQQHO IURP WKH 6DQWD $QD 3ROLFH 'HSDUWPHQW 2UDQJH &RXQW\ 'LVWULFW <br />Attorney¶s Office, and Orange County Sheriff-Coroner¶s Department. <br />During her briefing, Supervisor Monreal advised us of the following: <br />On June 25th, 2021, at approximately 1119 hours, Santa Ana Jail Correctional Officer E. Gonzalez #3366, transmitted <br />over the radio that he needed medics and additional correctional staff to respond to Module 4C. <br />As additional medical and correctional staff were responding, Correctional Officers Gonzalez and R. Zavala #2450 <br />were in inside of Cell 17, attempting to render medical aid to the inmate prior to the arrival of medical staff. <br />The inmate in Cell 17 was identified as Darryle Marcell Samuel. Correctional Officers Gonzalez and Zavala rendered <br />DLG WR 6DPXHO EXW KH ZDV XQUHVSRQVLYH $W DSSUR[LPDWHO\ KRXUV PHGLFV IURP WKH 2UDQJH &RXQW\ )LUH $XWKRULW\ <br />Station 76, arrived on scene and informed Supervisor Monreal that Samuel was deceased and would not be <br />transported from the scene. <br />3UHOLPLQDU\ LQGLFDWLRQV VXJJHVWHG WKDW 6DPXHO FRPPLWWHG VXLFLGH LQ KLV FHOO E\ KDQJLQJ KLPVHOI <br />&RUUHFWLRQDO 6XSHUYLVRU 0RQUHDO WKHQ FRQFOXGHG WKH EULHILQJ , ZDV DVVLJQHG WR FRQGXFW FDQYDVVLQJ LQWHUYLHZV RI WKH <br />medical staff that were involved and inmates within Module 4C. I conducted the interviews with Detective G. <br />Moroyoqui #2727 and Orange County District Attorney¶s Office Special Assignments Investigator C. Brower. <br />1435 hours: <br />I checked into the crime scene with Correctional Officer Godinez. <br />1450 hours: <br />Investigator Brower, Detective Moroyoqui, and I conducted interviews with the following medical staff: <br />SAPD Personnel:Sarabia, Melecio 2565 <br />Involvement:Report Approver <br />Santa Ana PD 2021-13541 6 of 7
