the booking area located on the first floor. <br />Sanchez stated she was in the booking intake area when she heard a radio transmission (10-49 4 Charlie). <br />As soon as she heard that call, she grabbed the emergency crash cart and proceeded to 4 Charlie with <br />other medical staff. As she was on her way, she heard another radio transmission advising 10-49 4 Charlie <br />man down. When she arrived at 4 Charlie cell 17, other medical staff were on scene. LVN Carolina Pena <br />and LVN Arianda Zamora were performing CPR on Samuel. <br />Sanchez saw that Samuel was in a supine position as Zamora and Pena were performing CPR. Samuel <br />was unresponsive. Samuel¶s limbs and fingertips were blue in color. Samuel was cool to the touch. <br />Sanchez said that an AED was applied. <br />Sanchez said that she observed red ligature mark on the front of Samuel¶s neck. She did not observed any <br />other injuries on his body. <br />Sanchez said she observed a bed sheet tied to a metal table inside the cell, which was approximately 5 feet <br />to 6 feet off the ground. <br />We concluded the interview. <br />1525 Hours <br />We then interviewed Ariadne Zamora. Zamora is an LVN and works for Naph Care. She has been <br />assigned to the Santa Ana Jail for approximately 6 years. <br />On today¶s date, she was assigned to the 3rd floor of the jail when she heard ³medical,´over the <br />radio. Zamora said she was the first to respond and say, ³Medical enroute.´Zamora said that the distress <br />on the officer¶s voice made her think that medical was indeed needed. Zamora grabbed their red medical <br />bag. Caroline Pena, Nurse Practitioner Rhea Marana and Zamora proceeded to 4 Charlie module. Pena <br />was ahead of her. As she was responding, she heard over the jail radio that they were needed at cell 17. <br />Upon her arrival, she observed Correction Officer Zavala and LVN Pena in the cell. <br />Zamora observed Samuel in a supine position. Pena was attempting to check for a pulse on Samuel. Pena <br />began to perform CPR on Samuel. Zamora alternated with Pena in performing CPR. An AED was placed <br />on Samuel. The AED device advised not to shock Samuel and they continued to perform CPR. Zamora <br />observed that Samuel was pale and his lips were bluish in color. <br />Zamora said she scanned the cell and observed a sheet hanging, which was tied to a top shelf/table. <br />Zamora stated Samuel looked familiar to her and did not know him. She observed a ligature mark on his <br />neck and did not observed any other injuries. <br />We concluded the interview. <br />1536 Hours <br />We interviewed Nurse Practitioner Rhea Marana. Marana works for Naph Care. She has been assigned to <br />the Santa Ana Jail for approximately 3 years. <br />On today¶s date, Marana was on the 3rd floor when she heard the LVN¶s (Zamora and Pena) say, ³Let¶s <br />go!´Marana was advised they were needed in 4 Charlie. When she arrived at 4 Charlie, Marana observed <br />Samuel in a supine position on the floor and Pena had begun to preform CPR. Marana observed Zamora, <br />Pena and Correction Officer Zavala performed CPR on Samuel. <br />Marana did not observe any injuries on Samuel. At first, she believed Samuel might have overdosed. <br />Narcan was administered to Samuel. An AED was used on Samuel. The AED advised not to shock <br />Samuel. They continued to perform CPR on Samuel until paramedics arrived. <br />Paramedics arrived and continued to perform life saving measures on Samuel. The paramedics were in <br />communication with Dr. Chen from Orange County Global Medical Center. At approximately 1147 hours, <br />Samuel was pronounced deceased. <br />Santa Ana PD 2021-13541 10 of 15
