EXHIBIT 2 <br /> FORM G <br /> PUBLIC UTILITIES COMMISSION <br /> OF THE <br /> STATE OF CALIFORNIA <br /> REPORT OF CHANGES AT HIGHWAY GRADE CROSSINGS AND SEPARATIONS <br /> MONTH OF December 2011 <br /> RAILROAD: SCRRA/METROLINK (CITY OF SANTA ANA-ORANGE SUBDIVISION) <br /> THE FOLLOWING CHANGES IN HIGHWAY GRADE CROSSINGS WERE MADE DURING THE YEAR 2011. The Common <br /> changes to approved GO-88-B include modifications of fencing and railing to accommodate field conditions. Added additional <br /> signage for the pedestrian swing gate and R 15-8"LOOK"per SCRRA Standard Drawings. Detectable warning tiles were trimmed <br /> to clear the #9 counter weights. Future location of the W10-9 "NO TRAIN HORN" sign on 17t" Street and Santa Clara to be <br /> installed by the City is shown on the as-built drawings. Field adjusted R8-8 to clear line of site for the flashers.(As-built drawings <br /> are attached). <br /> Nature of Change and Reference to Public Authority <br /> Crossing No. County Date Effective <br /> 1 101OR-173.6 Orange Fairhaven Avenue Grade Crossing - Implemented safety <br /> DOT 026697B improvements as authorized by GO-88-B letter dated <br /> 07/23/2009. <br /> 1. Street was closed into a Cul-De-Sac. <br /> 2 101OR-174.2 Orange Santa Clara Avenue Grade Crossing - Implemented <br /> DOT 02698H safety improvements as authorized by GO-88-B letter <br /> dated 07/23/2009. Modification to the original GO-88-B <br /> y are as follows: <br /> 1. Added W10-2R and W10-2L signs. <br /> 2. Added R3-1 and R13-A signs "No right turn <br /> and No right turn on Red' <br /> 3. Added W10-1 signs"RR symbols". <br /> 4. <br /> 3 101OR-174.7 Orange 171" Street Grade Crossing - Implemented safety <br /> DOT 026699P improvements as authorized by GO-88-B letter dated <br /> 07/23/2009. <br /> 4 101OR-175.1 Orange Santa Ana Boulevard Grade Crossing - Implemented <br /> DOT 026702V safety improvements as authorized by GO-88-B letter <br /> dated 07/23/2009. Modification to the original GO-88-B <br /> are as follows: <br /> 1. City of SJC modified median island and striping <br /> on the approaches to the crossing. As a result <br /> of this change median was widened, a #9 <br /> median gate for the eastbound traffic was <br /> added, and the traffic striping was changed to <br /> accommodate the City's improvements. <br /> 5 101OR-175.4 Orange 4th Street Grade Crossing - Implemented safety <br /> DOT 026703C improvements as authorized by GO-88-B letter dated <br /> 07/23/2009. <br /> December 9, 2011 Page 1 of 2 Prepared by: M.
