S.A. 12-031 <br />Investigator: TOM CONKLIN Approved By: <br />Date of Report: 12/17/12 Date: 12/17/12 <br />Page 1 of 1 <br /> <br /> <br /> <br />CASE NAME: LEON, IGNACHIO JR. <br />CLASSIFICATION: OFFICER INVOLVED SHOOTING (NON-FATAL) <br />TYPE: INITIAL RESPONSE <br />AGENCY: SANTA ANA POLICE DEPARTMENT <br /> <br />On Sunday, December 16, 2012, at approximately 0100 hours, Orange County District <br />Attorney (OCDA) Supervising Investigator RON SEMAN contacted me regarding an officer <br />involved shooting incident, involving the Santa Ana Police Department (SAPD). SEMAN <br />requested I respond to 506 East Washington Avenue, in the city of Santa Ana, and assist with <br />the investigation. <br />At approximately 0230 hours, I arrived at the location and met with investigators from <br />the Orange County District Attorney’s Office and from the Santa Ana Police Department, who <br />were present to assist in the investigation. <br />At 0245 hours, Santa Ana Police Sergeant JAY MILLER conducted a briefing of the known <br />circumstances surrounding the incident for the personnel involved in the investigation. <br />Following the briefing, OCDA Supervising Investigator RON SEMAN assigned me to assist <br />OCDA Investigator ED WOOD in scene responsibilities.
