SA 12‐022 <br />Investigator:  Approved By: <br />Date of Report:9/10/12 Date: 09/10/12 <br />Page 2 of 2 <br /> observed what she described as apparent entrance/exit wound to <br />ALMANZA’s right deltoid area and a  laceration to his right flank area.  According to  , <br />attending physicians were concerned with perfusion to ALMANZA’s right arm as they were <br />unable to detect brachial or radial pulses.  Additionally, physicians were able to palpate what <br />they believed  may be bullet fragments. <br /> On September 10, 2012, at approximately 0146 hours, ALMANZA was transferred  to <br />Operating Room #16 to undergo surgery due to the lack of blood perfusion to his right arm. <br />  did not have any further information to provide.
