S.A. 12-022 <br />Investigator: ANDREW ROSS Approved By: <br />Date of Report: 01/02/13 Date: 01/03/13 <br />Page 25 of 30 <br />On September 10, 2012, at approximately 0022 hours, Orange County Fire Authority <br />(OCFA) Unit T76, B9, Medic Unit 77 and Care Ambulance Service, unit CAR283, responded to <br />the shooting incident scene. <br />The following OCFA personnel responded to this incident: <br />• MARQUEZ Captain Unit T76 <br />• GRADY JOHN TIMOTI Engineer Unit T76 <br />• JOHN CLOUGHEN Paramedic Unit T76 <br />• PARIET HERNANDEZ Firefighter Unit T76 <br />• JOHN LOPEZ Paramedic Unit M77 <br />• RYAN TAYLOR Paramedic Unit M77 <br />At approximately 0028 hours, T76 arrived and fire personnel were directed to the <br />incident location by Santa Ana Police officers on scene. <br />When Paramedic JOHN CLOUGHEN entered the scene, he observed ALMANZA on the <br />ground, supine, shirtless and not handcuffed. He noted apparent blood on and around <br />ALMANZA and noticed ALMANZA appeared to be experiencing respiratory distress. <br />CLOUGHEN began to assess ALMANZA’s condition. CLOUGHEN believed ALMANZA <br />suffered gunshot wounds to the right chest, shoulder, arm and a deep graze to his right deltoid. <br />ALMANZA was uncooperative and refused to answer any questions asked by fire personnel. <br />At 0032 hours, ALMANZA was placed in the back of a Care Ambulance and transported <br />to the University of California, Irvine Medical Center (UCIMC), located at 101 The City Drive, in <br />the city of Orange. <br />While in the back of the ambulance, it was learned that ALMANZA did not suffer a <br />gunshot wound to the chest. An intravenous (IV) line was established in ALMANZA’s left arm <br />and he was provided supplemental oxygen.
