· OCDA Investigator J. Hoover (Orange County District Attorney’s Office) <br />· Freddie Deangelo Washington <br />NARRATIVE: <br />Thursday, January 16, 2025: <br />While at my desk, Detective Sergeant M. Lemmon #3174 notified me of an in-custody death investigation. He <br />requested I respond to the Santa Ana Police Jail and assist with the investigation. <br />1615 hours: <br />Santa Ana Police Correctional Supervisor E. Villegas #3098 briefed members of the Santa Ana Police Homicide Unit, <br />Santa Ana Police Professional Standard Inspectors, Orange County District Attorney Investigators, and Orange <br />County Sheriff Forensic Unit personnel. <br />Supervisor Villegas said Santa Ana Police Correctional Officers Freddie Washington when he collapsed. Washington <br />would be transported to St. Joseph’s hospital where he would be pronounced deceased. <br />1623 hours: <br />Correctional Supervisor Villegas ended the briefing. Santa Ana Police Correctional personnel walked us through the <br />jail where the incident occurred. <br />I was later tasked with responding to St. Joseph's Hospital along with District Attorney Investigator J. Hoover. <br />1635 hours: <br />I left the Santa Ana Jail and responded to St. Joseph's Hospital. <br />1706 hours: <br />I arrived at St. Joseph's Hospital and met with Investigator Hoover and Orange County Sheriff's Deputy Coroner L. <br />Martin ##11543. <br />1743 hours: <br />Investigator Hoover and I entered St. Joseph's Hospital through the emergency room entrance and contacted <br />Corporal T. Engle #3629 and Sergeant B. Mercer #3077, standing outside Emergency Room #9. Corporal Engle <br />advised Washington was inside Emergency Room #9. The entry to the room consisted of a sliding glass door with an <br />extended curtain on the interior. The curtain prevented a visual inside the room. <br />1746 hours: <br />Orange County Forensic Specialist L. Perlin #11846 arrived. Investigator Hoover, Forensic Specialist Perlin, Deputy <br />Coroner Martin, and I entered Emergency Room #9. Deputy Coroner Martin and Forensic Specialist Perlin took <br />photographs and conducted an initial examination of Washington’s body. Deputy Coroner Martin collected <br />Washington's clothing and property which included his New York identification ). The <br />identification contained Washington's photograph and the following information: <br />1. Name: Freddie Deangelo Washington <br />2. Address: <br />3. DOB: 02/29/88 <br />4. Physical Description: 5'09" tall and brown eyes <br />1815 hours: <br /> from Traditional Funeral Services arrived. <br />1824 hours: <br />Santa Ana PD 2025-01082 Report by Corona, Gerardo 3319 2 of 3
