At 0616 hours, an inmate headcount was conducted. <br />At 1048 hours, Correctional Officer C. Carrillo #3918 and Correctional Officer J. Alston #3663 offered to allow <br />Washington to stretch his limbs and drink water. Washington accepted the water. <br />At 1316 hours, Washington was removed from the restraint chair. He was then handcuffed behind his back and <br />seated in a chair in the intake/booking area. <br />At 1326 hours, Washington was medically cleared by Registered Nurse for transport to the Orange <br />County Jail. Another arrestee, identified as a ), was grouped with Washington for <br />transportation to the Orange County Jail. <br />At 1328 hours, Washington and were escorted by Correctional Officers Carrillo and Alston to the elevators. <br />At 1329 hours, they all entered elevator H2 and Washington suddenly fell backwards in an apparent fainting spell or <br />seizure. Correctional Officers Carrillo and Alston helped guide Washington to the ground. Washington was then <br />removed from the elevator for medical assistance. <br />At 1330 hours, Nurse responded to provide medical aid to Washington. <br />At 1333 hours, Washington suddenly regained consciousness and became uncooperative with correctional officers <br />and medical staff. <br />At 1334 hours, Washington was placed back into a restraint chair and a spit mask was applied. Washington was <br />conscious and breathing. <br />At 1338 hours, Washington remained in the restraint chair and he was wheeled to detox cell D1 for further medical <br />evaluation. During the medical evaluation, it was discovered that Washington was not breathing and he was <br />immediately removed from the restraint chair. CPR was then performed on Washington. <br />At 1340 hours, Orange County Fire Authority was requested for medical aid for Washington. <br />At 1347 hours, medics from the Orange County Fire Authority arrived on scene. They continued life saving measures <br />on Washington. <br />At 1417 hours, Washington was transported to St. Joseph’s Hospital by ambulance. <br />At 1438 hours, Washington was pronounced deceased by the treating emergency room doctor at St. Joseph’s <br />Hospital. <br />At the conclusion of the briefing, Santa Ana Police Department Correctional personnel walked us through the jail <br />where the incident occurred. After observing where the incident occurred, I was assigned as the lead investigator from <br />the Santa Ana Police Department for this investigation. Orange County District Attorney’s Office Investigator T. Toth <br />was assigned as their lead investigator. <br />Santa Ana Police Department Detective Corporal B. Speakman #3360 was assigned to document the scene of where <br />the incident occurred with Orange County District Attorney’s Office Investigator C. McCarthy. For further details, refer <br />to Detective Corporal Speakman’s supplemental report under this case number. <br />Santa Ana Police Department Detective F. Gomez #2706 was assigned to collect surveillance video of the <br />incident. For further details, refer to Detective Gomez’s supplemental report under this case number. <br />Santa Ana Police Department Detective J. Capacete #2998 was assigned to interview a with Orange <br />County District Attorney’s Office Investigator McCarthy. For further details, refer to Detective Capacete’s supplemental <br />report under this case number. <br />Santa Ana Police Department Detective G. Corona #3319 was assigned to respond to St. Joseph’s Hospital with <br />Orange County District Attorney’s Office Investigator J. Hoover to document the processing of Washington’s body. For <br />further details, refer to Detective Corona’s supplemental report under this case number. <br />I returned to the Santa Ana Police Department and continued the investigation. <br />This concluded my actions for the investigation of this case on this date. <br />Detective C.
