Thursday, January 16, 2025: <br />While at my designated workstation at the Santa Ana Police Department, Detective Sergeant M. Lemmon #3174 <br />notified me of an in-custody death that had occurred. Detective Sergeant Lemmon requested that I respond to the <br />Santa Ana Detention Facility to assist with the investigation. <br />1615 hours: <br />Santa Ana Police Department Correctional Supervisor E. Villegas #3098 conducted a briefing with Santa Ana Police <br />Department Homicide Detectives, Santa Ana Police Department Professional Standards Unit Inspectors, and <br />Investigators from the Orange County District Attorney’s Office, regarding the in-custody death, timeline of events and <br />actions of personnel involved. <br />At the conclusion of the briefing, I was assigned to interview Registered Nurse with Investigator Dave <br />Melnyk. Below is a summary of the interview. <br /> stated he has been a registered nurse for approximately 2 years and has worked at the Santa Ana jail since <br />July of 2024. currently works Wednesdays – Fridays from 0600 – 1830 hours. Today, at approximately 1328 <br />hours, checked Freddie Washington’s vitals which were normal. At that time, Washington showed no signs <br />of distress. Shortly thereafter, heard a medical aid request via radio near the elevator. responded <br />and observed Washington handcuffed on the floor. described Washington as being aggressive, fighting and <br />combative. Washington was placed onto a restraint chair and taken to a holding cell. checked Washington <br />again and noticed he was not responsive and had no pulse. Washington was given Narcan and custody personnel <br />provided CPR.
