Required Documentation and Justification Narrative <br /> All supportive services must be fully documented and clearly connected to the participant's <br /> employment plan.The Career Navigator must document the request in the participant's service <br /> plan and case notes in Casebook and ensure the file demonstrates how the service supports <br /> participation in program activities,training,Work Experience (WEX),job search,or employment. <br /> Each supportive service must include: <br /> • Completed Supportive Service Request Form <br /> • Written justification explaining the employment barrier <br /> • Approval signature <br /> • Proof of payment/purchase <br /> • Proof of receipt by participant,if required by contract <br /> o One-Time Waiver:Should a participant lose his/her receipt,they will be required to <br /> complete an applicant statement with all pertinent information.Should a <br /> participant lose a subsequent receipt,they will be penalized by not being able to <br /> obtain another supportive service for a 30-day period. <br /> • Case note and service entry in Casebook,and in any external reporting platform required by <br /> the applicable contract <br /> The justification narrative must explain: <br /> • The employment barrier <br /> • Why the barrier prevents participation or employment <br /> • How the requested service resolves the barrier <br /> • The expected employment activity or engagement following receipt of the service <br /> Types of Supportive Services <br /> Allowable supportive services vary by contract.They may include,but are not limited to: <br /> Transportation <br /> • Gas cards or public transit passes <br /> • Rideshare assistance when no reasonable transportation alternative exists <br /> • Automotive repair assistance necessary for transportation to employment or program <br /> activities (case-by-case review) <br /> • Other transportation needs such as car registration,insurance payment,etc. <br /> Employment Preparation and Work-Related Needs <br /> • Interview clothing or required work attire not covered through wardrobing <br /> • Work shoes or safety equipment required for employment or WEX <br />
