<br /> <br /> WC required if consultant has N-2002-045-006 <br /> employees <br /> O S AP D C°-) OCT 2 7 "Z01Q <br /> Lori Q jaws-, NINTH AMENDMENT TO <br /> A - 0, Le?.rY CONSULTANT AGREEMENT <br /> THIS NINTH AMENDMENT TO CONSULTANT AGREEMENT is entered <br /> into on October & , 2010, by and between DANIEL ACHATZ ("Consultant") and the <br /> City of Santa Ana ("City"). <br /> RECITALS: <br /> A. The parties entered into Consultant Agreement N-2002-048, dated February 28, 2002, <br /> (hereinafter "said Agreement") by which Consultant has provided counseling and <br /> instructional services to the inmates at the Santa Ana City Jail. <br /> B. In accordance with the terms and conditions of said Agreement, the parties wish to <br /> renew said Agreement for an additional one-year period. <br /> WHEREFORE, in consideration of the covenants contained in said Agreement, and <br /> subject to all the terms and conditions of said Agreement, as previously amended, except <br /> those amended in this Ninth Amendment to Consultant Agreement, the parties agree as <br /> follows: <br /> 1. Section 3, TERM, shall be amended to extend the termination date one year, from <br /> June 30, 2010, through June 30, 2011. In order to provide continuous uninterrupted <br /> service, all services provided by Consultant since July 1, 2010, shall be.included <br /> within the Scope of Services of this Agreement. <br /> 2. Except as herein amended, all terms and conditions of said Agreement, as previously <br /> amended, shall remain in full force and effect. <br /> IN WITNESS WHEREOF, the parties hereto have executed this Ninth Amendment to <br /> Consultant Agreement on the date and year first written above. <br /> ATTEST: CITY OF SANTA NA <br /> MARIA D.
