Distribution Data - written Plan Administrator's (or authorized Designee's) direction to <br /> commence distribution processing, which contains the following items of Participant <br /> information: <br /> <br /> (A) Agency name <br /> (B) Participant's legal name <br /> (C) Participant's social security number <br /> <br /> (D) Participant's address <br /> (E) Participant's phone number <br /> (F) Participant's birthdate <br /> <br /> (G) Participant's condition of eligibility <br /> (H) Participant's effective date of eligibility <br /> (I) Signed certification of distribution eligibility from the Plan Administrator, or <br /> authorized Designee <br /> <br /> 3.
