Attachment A <br />Reimbursement Form Display Dashboard <br />• Table view built as a MindTouch Template will be marked as semi public to <br />prevent end user tampering <br />• Table columns include: <br />o Name of person <br />o Name of class <br />o Data of class <br />o Total Cost of class <br />o Status of reimbursement <br />• Status of reimbursement can be changed directly from the dashboard. Status <br />options include: <br />o Pending <br />o Rejected <br />o Approved <br />• All new reimbursements will default to a status of Pending <br />• The Reimbursement Dashboard will include sortable table columns for the <br />provided columns <br />• The Reimbursement Dashboard will not include pagination <br />• Upon submitting the reimbursement form the user will be redirected to their <br />newly created reimbursement page <br />• Only the reimbursement status can be changed by administrators <br />Grant Entry Form Plugin <br />• The Grant Entry Form will be created as a MindTouch Plugin <br />• The form will include the following inputs: <br />o Text Input: User - pre-populated from MindTouch <br />o Text Input: Title <br />o Timeline <br />¦ Text Input: Start Date - Date picker <br />¦ Text Input: End Date - Date picker <br />o Text Input: Cost <br />0 2 File uploads <br />o Page location picker <br />o Textarea Input: Grant Details <br />• Will include error messages for predefined required fields <br />• Will not include validation for input formatting <br />18 of 20
