THE PREVENTATIVE MAINTENANCE AGREEMENT <br />Johnson Controls is pleased to present you with the follow(ng proposal for consideration. We are pleased <br />to provide you with a premium level service agreement which Includes all of your maJor HVAC equipment <br />to be serviced on a quarterly basis. Today, you can count on a Johnson Controls maintenance agreement <br />to help: <br />� Keep occupants comfortable and productive. <br />� Minimize energy consumption and maintain equipment efficiency. <br />� Prevent system outages. <br />� Provide you with the Information you need to make cost - effective decisions about system repair <br />or equipment replacement. <br />Johnson Controls will provide the following professional mainlenance services on the HVAC <br />system specified on the attached "Inspection Summery." <br />� Preventive Maintenance and Inspection Services —Under this agreement, Johnson Controls <br />will provide preventive maintenance and inspection labor, associated materials and travel labor <br />and expenses, required to: <br />• Conduct 4 preventative maintenance visits per year. <br />• Perform preventive maintenance In accordance with manufacturer recommendations, <br />equipment run - hours, application demands, environmental factors and the experience of <br />Factory technicians and service managers. All work will be performed during normal <br />Johnson Controls working hours unless otherwise indicated In the Special Service <br />section of this agreement. <br />• Identify defects, failed or doubtful components, water or oil leaks, flue gas leaks, <br />excessive vibration, noisy or Improper operation. <br />• Inform customers of preventive maintenance progress, Inspection findings and <br />applicable maintenance and repair options. <br />• Recommend cost - effective repairs, applicable retrofit options and equipment <br />replacement alternatives. <br />� Repairs and Unscheduled Service Calls -- Thls agreement does not Include system repairs, <br />parts Installation or service calls made at the customer's request. <br />� Emergency Service Response -- Service technicians will respond to the customer's request for <br />emergency service 24 hours a day seven days a week for coils going to our Service Number at <br />866 -819 -0230. Emergency response time shall be 4 hours. <br />� For Services Not Included under This Agreement — Johnson Controls will furnish labor, <br />overtime or emergency labor, repair parts and components, regulated material recovery and <br />disposal services at PREFERRED CUSTOMER RATES. <br />� Labor end Material Discounts —all maintenance contract customers receive a labor discount of <br />i O% on our published labor rates per hour and a 1 O %discount on parts. <br />� Primary Technician — Johnson Controls will assign a Primary &Secondary technlclan for the <br />mechanical scope of this agreement.
