GROUNDSILANDSCAPE MAINTENANCE CONTRACT <br />SPECIAL PROVISIONS <br />1. GENERAL_ CONDITIONS <br />A. FUNCTIONS AND RESPONSIBILITIES <br />1. <br />The Executive Director of Parks Recreation and Community Services ("Director') or his Designee <br />shall accept/reject materials and workmanship, and may make minor changes to the scope of <br />work and specifications as set forth herein which do not involve extra cost to the Contractor. <br />2. Contractor <br />a) 24 -Hour Contact Number <br />The Contractor shall provide the Director with a telephone number at which a contact <br />person for the Contractor may be reached 24 hours a day. The contact person shall <br />be required to respond back to the Director or his representative within 1 hour. The <br />contact person shall have the appropriate knowledge and/or authority to take <br />appropriate action in response to any emergency situation that may arise involving <br />the contractor's responsibilities hereunder. <br />b) Submittals <br />Prior to performing the services as provided herein, Contractor shall submit to the <br />Director for approval: 1) a detailed annual, monthly and weekly work schedule; 2) <br />names and titles of all persons working on the project; and, 3) lists of equipment, <br />materials and/or chemicals to be used on the project. The information contained in <br />the above referenced documents shall be updated with any new information as <br />available. The Director shall be immediately notified of any changes to the above <br />information. <br />c) Uniforms and Vehicle Identification <br />The Contractor shall provide to all of its field personnel a standard uniform including, <br />but not limited to, uniform company hats, collared work shirts, pants, and ANSI <br />approved safety shoes. All uniforms will be marked with some form of company logo <br />or other form of identification. <br />All vehicles and equipment on the project site shall be in an operable and working <br />condition, with a clean appearance without visible damage, dirt, graffiti etc. In <br />addition, all vehicles shall have the company's name clearly identified on the right <br />and left side doors. <br />Exhibit A -2 <br />
