EXHIBIT D <br />CONSULTANTS <br />In the event that Contractor shall enter into consultant and /or professional service agreement for any <br />services provided under this Agreement, the parties to any such agreement, and the services they <br />are to provide, shall be identified below. <br />Contractor shall provide SBWIB, Inc. with current copies of any consultant and /or professional <br />services agreements with the individuals listed below. Said agreements shall specify compliance <br />with terms and conditions of the primary agreement with SBWIB, Inc. <br />Adjustments to this Exhibit may be made by approval of the SBWIB, Inc., Attorney without <br />amendment, however, prior to any change in, or additions to, the list of consultants contained <br />herein, Contractor shall notify SBWIB, Inc. and provide copies of sub - agreements and other <br />required documents. <br />Consultants performing services which may involve driving must provide evidence of insurance <br />(insurance certificates) at the level required and with additional insured endorsements. <br />CONSULTANT SERVICES TO BE PERFORMED <br />.. <br />
