<br /> <br /> <br /> INSURANCE ti(A h REQU REO N-2005-123-005 <br /> WORK ~44Y PROCEF'-) <br /> CLERK Of L')~ <br /> SIXTH AMENDMENT TO <br /> GATE; CONSULTANT AGREEMENT <br /> <br /> <br /> THIS SIXTH AMENDMENT made and entered into on this 1st day of July, 2012, by and <br /> between Cesar Vargas (hereinafter "Consultant"), and the City of Santa Ana, a charter city and municipal <br /> corporation of the State of California (hereinafter "City"). <br /> <br /> RECITALS <br /> A. The parties entered into Agreement #N-2005-123, dated October 17, 2005, (hereinafter "said <br /> Agreement") by which Consultant has provided translation/interpretation services. <br /> <br /> B. In accordance with the terms and conditions of said Agreement, the parties wish to extend the <br /> term of said Agreement to provide continuous, uninterrupted service. <br /> <br /> NOW THEREFORE, in consideration of the mutual and respective promises, and subject to all the <br /> terms and conditions set forth, the parties agree as follows: <br /> <br /> 1. The TERM of said Agreement shall be extended for an additional one-year period, <br /> through June 30, 2013. <br /> 2. City agrees to pay, and Consultant agrees to accept as total payment for its services, the <br /> hourly rate of $95.00, with a two hour minimum, which includes up to thirty (30) minutes <br /> of preparation, familiarization and review time, as referenced in Section 1 b of original <br /> Agreement. The total compensation to be paid pursuant to this Agreement shall not <br /> exceed $10,000.00 during the term of this renewal period. <br /> <br /> 3. Except as hereinabove amended, all terms and conditions of said Agreement <br /> shall remain in full force and effect. <br /> <br /> IN WITNESS WHEREOF, the parties hereto have executed this Sixth Amendment to <br /> Consultant Agreement on the date and year first written above. <br /> <br /> CITY OF SANTA ANA <br /> ATTEST: <br /> <br /> <br /> <br /> ARIA D.
