CITY OF SANTA ANA <br />PUBLIC WORKS AGENCY <br />CONTRACT CHANGE ORDER <br />Project Number Project No. 12-7527; Jerome Park Improvements Change Order Number 2 <br />To Excel Pavina Corri Contractor <br />You are hereby directed to make the herein changes from the plans and specifications or do the following described work not included in the plans and <br />specifications on this contract. <br />NOTE: THIS CHANGE ORDER IS NOT EFFECTIVE UNTIL APPROVED BY THE CITY COUNCIL OR CITY MANAGER. <br />Unless otherwise stated, rates for rental of equipment cover only such time as equipment is actually used and no allowance will be made for idle time. <br />Change requested by <br />PUBLIC WORKS AGENCY <br />EXTRA WORK AT AGREED PRICES <br />Remove and dispose of the existing non-functioning light poles and fixtures adjacent <br />to the basketball courts per City of Santa Ana PCO #03 dated 11/28/2012. <br />AGREED COST = $ 2,141.61 <br />2. Install conduits, wiring and make the connections to enable the new lighting circuits <br />to be controlled through the existing Musco° control cabinet per City of Santa Ana <br />PCO #03 dated 11/28/2012. <br />AGREED COST = $ 3,747.81 <br />3. Remove the existing low voltage irrigation control wiring from the line voltage conduit <br />and install, via direct bury method, from the valve location to the existing irrigation <br />controller located in electrical room per City of Santa Ana PCO #03 dated <br />11/28/2012. <br />AGREED COST = $ 5,428.97 <br />4. Remove the existing non-functioning light fixtures in the basketball court, modify the <br />existing poles to accept new lights and install eight new LED light fixtures. Locate <br />and intercept the existing conduit and install new wiring from the new basketball <br />court lights to the existing Musco°controller per City of Santa Ana PCO #03 in <br />basketball court area. <br />AGREED COST = $ 14,923.42 <br />Page 1 of 2
