City of Santa Ana <br />Scope of Work <br />Name of Organization Share Our Selves <br />Name of Funded Program SOS Social Services, Rental' Assistance Program <br />Annual Accomplishment Goal <br />1. Total number of unduplicated households (Santa Ana and Non-Santa Ana Residents) anticipated to be served by the <br />funded pro ram, named above, during the 12-month contract period. <br />70 Households <br />II. Number of unduplicated Santa Ana residents expected to be served by the funded program during the 12-month <br />contract period., <br />40 Households <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Only the <br />viewable space will print. <br />SOS will administer rental assistance supported by ESG funding to Santa Ana residents to include: short to <br />medium term rental subsidies (1-3 months), security deposits and last month/past due rent. All Santa Ana <br />residents receiving ESG fudning will be enrolled in Case Management which will consist of goal setting and <br />promoting self-sufficiency to produce sustainable housing. <br />Number of persons is estimated based on a each househould averaging a family of four (4). <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana Households to be served by the funded program during the 12-month <br />contract period per quarter. (Enter number of new Santa Ana households served each quarter. If they were served in <br />quarter 1 do not count them again in quarter 2) <br />Quarter 1: July 1 - September 30 10 Households <br />Quarter 2: October 1 - December 31 10 Households <br />Quarter 3: January 1 - March 31 10 Households <br />Quarter 4: April 1 -June 30 10 Households <br /> 40 Total unduplicated Santa Ana households to be served. <br />Schedule of Invoicing <br />Estimate the amount of grant funds to be requested during the 12-month contract period on a quarterly basis. <br />Quarter 1: July 1 - September 30 $10,698.25 <br />Quarter 2.
